Sourcing & contracting

OC Reilly is the sourcing and contracting partner for three Maine Critical Access Hospitals, running their procurement, equipment and service contracts, and group-purchasing revenue programs under direct contracts with each. A member-governed collaborative built on Protoco Collaboration Group (PCG) lets the hospitals share system-scale infrastructure while keeping full local control.

Independent rural hospitals carry the same supply, equipment, and contracting costs as large systems, but without the scale to negotiate against them — and most consolidation models fix that math only by absorbing the hospital. OC Reilly built a different model in Maine: under direct contracts with Northern Maine Medical Center, Mount Desert Island Hospital, and Houlton Regional Hospital, the firm runs day-to-day procurement while PCG provides shared infrastructure underneath. Each facility keeps full operational independence. The partnership has since extended through the Maine Hospital Association and a shared Medical Equipment Management initiative.

The challenge

These hospitals needed leverage they couldn't generate alone — without giving up the independence that defines them. Past regional collaboratives had failed them on exactly that trade-off.

  • System-scale leverage no single hospital could reach alone

  • Strong reluctance to surrender local control

  • Procurement decided facility-by-facility, leaving savings unclaimed

  • Past collaboratives failed without funding or accountability

Our solution

OC Reilly made value, not control, the price of entry. A portion of its compensation is tied to the savings it delivers.

  • Direct contract with each hospital.

  • Compensation tied to documented savings.

  • Member-governed collaborative on a shared PCG shell.

  • Daily procurement and group-purchasing managed throughout.

Key results & impact

OC Reilly has delivered measurable, documented savings across the three-hospital partnership while keeping every facility independent — and built collaborative infrastructure durable enough to support additional shared initiatives.
